How to Request an Accommodation
In accordance with California Rules of Court, rule 1.100 , requests for accommodations must be made as far in advance as possible and no fewer than five (5) court days before the requested date. The Court may waive this requirement at its discretion but may be unable to accommodate untimely requests.
Form MC-410: Request for Accommodations by Persons with Disabilities
Requests may also be submitted in another written format or made orally.
When requesting an accommodation by telephone, please be prepared to provide the following information:
- Name and address of the person requiring the accommodation
- Name of the court facility and department or courtroom number
- Case name
- Case number
- Type of proceeding (for example: civil, small claims, unlawful detainer, criminal, traffic, juvenile dependency, juvenile delinquency, family law, or probate)
- Date the accommodation is needed
- Impairment necessitating the accommodation
- Type of accommodation requested
All requests must be submitted to the ADA Coordinator using the contact information listed above.
Americans with Disabilities Act Grievance Procedure
Complaints alleging discrimination based on disability may be filed using the court’s ADA Grievance Procedure , established in accordance with the Americans with Disabilities Act of 1990.
Do not use this grievance procedure if you filed a Request for Accommodation (Form MC-410), your request was denied, and you wish to appeal. Appeals are governed by California Rules of Court, rule 1.100(g).
