- Complete all required fields.
- If not typed, print neatly in blue or black ink.
- Sign and date forms where required (by petitioner(s) and/or attorney of record).
Submit documents by eFiling. After acceptance, stamped documents are available to download for 30 days in the eFiling Portal under “Filing History.” After that, use the San Joaquin Superior Court Public Portal or contact Records Management.
Or submit the original plus one set of copies for your records. Include a self-addressed stamped envelope (SASE) with enough postage if you want copies returned by mail. If you do not include a SASE, copies may be placed in the pickup cabinet at the Security Desk in Stockton. Documents not picked up within 30 days may be destroyed.
Pay the required filing fee. View current fees at Court Fee Schedule.

If you cannot afford court fees and costs, you may request a fee waiver by completing form FW-001. See the Guardianship and Conservatorship Fee Waiver Form Packet.

Court proceedings are in English. If you need an interpreter, you may request one by completing and filing Judicial Council form INT-300 (Request for Interpreter—Civil).
