Submitting your forms for filing

  • Submit all forms required for your specific situation.
  • All information must be completed.
  • Forms must be dated and signed by the petitioner or attorney of record.
  • Documents must be typed or neatly handwritten using blue or black ink.
  • You may file by eFiling. Accepted eFiled documents are available for download for 30 days in the eFiling portal.
  • You may also submit originals plus one copy. Include a self-addressed stamped envelope (SASE) if you want copies returned by mail. Documents not picked up within 30 days may be destroyed.
  • Filing fees apply. View current fees at www.sjcourts.org/forms-filing/court-fee-schedule .
  • If you cannot afford the filing fee, you may request a waiver using form FW-001. See the Guardianship and Conservatorship Fee Waiver Form Packet .

Interpreter needs

All court proceedings are conducted in English. If you need an interpreter, you may request one by completing and filing Judicial Council form INT-300 (Request for Interpreter) .
