Petition to Change Name – General Instructions

You may not need all of the forms listed above. Carefully review the Instructions for Filing a Petition for Change of Name (NC-100) or other applicable instruction forms before completing your paperwork.

Submitting your forms for filing

  • Provide all required forms for your situation.
  • All information must be completed.
  • If handwritten, print legibly using blue or black ink.
  • All forms must be dated and signed by the petitioner.
  • Submit the original plus one copy for your records. Include a self-addressed stamped envelope (SASE) with adequate postage to have copies returned. Documents not picked up within 30 days may be destroyed.
  • Provide the correct filing fee for your petition. Current fees are listed at Court Fee Schedule.

If you cannot afford to pay court fees, you may request a fee waiver by completing form FW-001. See the Fee Waiver Form Packet for additional information.

There is a separate fee for publishing the Order to Show Cause in a newspaper. Approved newspapers are listed here: Approved newspapers for publication .

Interpreter needs

All court proceedings are conducted in English. If you need an interpreter, you may request one by completing Judicial Council form INT-300 – Request for Interpreter .

Proof of publication

Confirm that the newspaper has filed a Proof of Publication with the Court before your hearing date.
