Form Packets

Effective July 6, 2021, the Clerk’s Offices no longer provide form packets for purchase. You are encouraged to print and complete any required forms before visiting the courthouse. Doing so may save time and reduce the need for additional visits.

Superior Court staff are prohibited from providing legal assistance or legal advice. Individuals representing themselves (In Pro Per) are responsible for determining which forms to file and ensuring all filings comply with applicable legal requirements.

All documents submitted to the Court must be properly completed and accompanied by the required filing fee or an approved fee waiver application. If you need assistance, consider consulting an attorney or reviewing self-help resources available through California Courts or the Self-Help / Pro Per Clinic.
